When you come for your appointments, please remember to bring the following

• Completed Patient Registration Form
• Referral letter from GP, family physician or other doctor
• Medicare card, DVA card, pension card
• Have your Private Hospital Insurance information with you
• Reports, X-rays, MRI’s, CT scans etc. and any other relevant information

Please note that limited PAID car parking is available on site at Kangan Drive. FREE on street parking is limited and signposted for the parking conditions.
